How to nominate a Care Recipient
Nominate someone who you think would benefit from home care services. Heaven’s Hands for Seniors will provide funding that can be used for 6-8weeks of home care services or care advocacy services. Eligibility for home care support by Heavens Hands for seniors is a two-part process.
Part 1 (Functional)
The individual being nominated must have a need for the services. Specifically, you must physically demonstrate that you have a needs For Hands on assistance in performing some or all activities of daily living.
Bathing - the individual needs help to properly bathe themselves
Eating/feeding - the individual needs help feeding themselves properly this includes making or preparing meals
Toileting - the individual needs help going to and from the bathroom as well as cleaning up after themselves properly
Transfers - the individual needs how to safely transfer in and out of their bed or a chair
Medication - the individual needs help preparing, and being reminded to take daily medication’s
Behavioral - the individual needs daily supervision to keep from harming themselves or others
Part 2 (Financial)
The individual being nominated, must demonstrate financial inability to afford home care services. Otherwise the individual must meet an income requirement of $2500 per month.
